  • When and How to Return 2020-2021 Flu Vaccine

    All flu vaccines will have expired on June 30, 2021. The flu vaccines cannot be adjusted as Expired until after their expiration date. Be sure to adjust all used and expired flu vaccines from your inventory in Florida SHOTS for the number of doses expired and for the number of doses administered.

    Perform an inventory adjustment by decreasing the number of doses expired and select "Expired” from the drop-down menu.  Perform the same steps for the doses administered and select "Doses Administered” from the drop-down menu.  After making these adjustments, go to Transaction History, pick flu for the Vaccine Category, and then click "Retrieve Matching Inv Records.” All flu inventory should be zeroed out. 

    Once all adjustments are made, click on "Report Returns/Waste.” At the end of each row pertaining to the flu adjustments, place a checkmark in the box before clicking the Submit button. This will redirect you to the Shipping Labels page where you will then click on the "Request Labels button.” On this page, you will need to verify the email address shown is valid. Now choose from the drop-down menu next to Nbr of Boxes, the number of return shipping labels you require. VFC does not provide the box(s). You will then click the "Complete” box before clicking the "Submit” button.

    After the return shipping label request has been made and exported, an email will be sent from UPS Quantum View [mailto:[email protected]] within 3 business days containing the shipping label for you to print out and attach to the box. You will then drop the box off at a UPS location or have the UPS delivery person take the box with them when making a delivery to your office. DO NOT request a UPS pick-up. You will be charged and not reimbursed.

  • UPDATE: Inventory Adjustment for COVID-19 Vaccine

    Florida SHOTS has been updated to allow providers to go into their inventory and adjust the expiration dates to the correct one for their COVID-19 vaccines. The easy-to-follow steps on how to complete the adjustment can be accessed HERE.

    You can now also complete inventory adjustments for expired COVID-19 vaccines that are in your inventory with the 12/31/2069 expiration date.  Access instructions HERE and in Spanish HERE.

  • Electronically Certifying DH 680 Form

    Florida SHOTS allows healthcare providers to create a Certified (electronically signed) Certificate of Immunization (DH Form 680) and a PIN that will allow parents and guardians to access the certified 680. The DH 680 form is legal and valid in the state of Florida for admittance to schools and daycare facilities.
